Applicants must submit proof of Continuing Competency including course outlines/objectives pursuant to Rule 490-4 and Board Policy.

Applicants must submit a Curriculum Vitae or Resume which   details their work history for at least the past two years.

If an applicant has not been actively practicing physical therapy for the past two or more years, he/she will be required to compete 1000 hours of supervised practice under a Training Permit.

Applicants must submit verification of licensure from all states in which the applicant has ever been licensed, regardless of current status.
