Applicants must submit Official evaluation and transcript from a credential evaluation organization approved by the Georgia Board of Physical Therapy.  Currently, the Board recognizes the following credentialing evaluation agencies:

Applicants must submit official transcript from college or university;  
     
Applicants must submit passing scores from the NPTE.

Applicants must submit verification of licensure from all states in which the applicant has ever been licensed, regardless of current status;

Applicants must pass the three (3) Language Proficient Examinations:  TSE, TOEFL, and TWE.

Applicants must submit verification of eligibility for licensure/certification/registration from your Country of residence. 

Applicants must compete the Board approved traineeship (See Rule 490-2-.04).
