• Submit completed application along with appropriate fee.  Applications and the fee schedule  may be found at http://www.sos.state.ga.us/cgi-bin/plbforms.asp?board=15 ;
•  Applicants must be graduates of a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited school.   Applicants must provide official transcript from the institution granting the entry level degree in physical therapy or physical therapy assistant indicating the date of graduate;

c)  Pass the National Physical Therapy Exam (NPTE); and
d)  Pass the Georgia Jurisprudence (Georgia Law) exam
