A10. The Board accepts address changes for pharmacists and interns submitted online, via letter, or by fax at 866-888-1176. A name change request must be submitted in writing accompanied by supporting legal documents. For pharmacy name or address changes, file an application with the Board. The application may be found at http://www.sos.state.ga.us/cgi-bin/plbforms.asp?board=03 . GDNA must conduct a site visit and submit approval to the Board prior to the name and/or address of the pharmacy being changed. |
