Please write the Board and request a duplicate card. Includeyour name, address, license number and the appropriate fee (see Fee Schedule found in the download forms link on the website).

You should receive your new pocket card within fifteen (15) working days.

There is a $25 fee to request a reprint of your license pocket card once name changes are made
