A1. Submit a completed application by examination, non-refundable fee, copy of birth certificate, and photo within one year.

If you currently hold a license in another, submit a state copy of the license and licensure verification from all states licensed.

All candidates for the Practical Examination must provide their own equipment and are required to bring the following to the examination:  speech audiometer, blank audiograms, and ear mold impression materials and equipment (including otoscope & ear light), and word list.
